The ambulance doors burst open. "Emergency! Male, nineteen years old. Severe hemoptysis. Possible respiratory failure." Doctors and nurses rushed Noah through the corridor. Elena stumbled after them, her legs trembling. "Please... I'm his sister." A nurse gently stopped her before the operating room. "You can't go any farther." "But…" "They need room to work." The doors swung shut. The red EMERGENCY light flickered on. And just like that... Elena was alone. Hospitals had a way of making time feel extremely slow. Each passing second felt longer than the last. She paced the waiting area, unable to sit still. Her blouse was stained with Noah's blood. She hadn't even noticed until now. Her hands shook as she tried to wipe it away. It wouldn't come off. She buried her face in her palms. "Please be okay..." An elderly woman sitting nearby quietly handed her a packet of tissues. Elena accepted it with a weak smile. "Thank you." Nearly two hours later... The emergency room doors finally opened. A middle-aged doctor stepped outside, removing his surgical gloves. "Miss Hart?" Elena jumped to her feet. "Yes! How is he? Is my brother okay?" The doctor's expression softened. "He's stable now." She let out a huge breath of relief. "Oh, thank God..." "But..." That single word made her heart stop. "The bleeding wasn't random." He motioned for her to follow him into a quieter consultation room. Once the door closed, he laid Noah's latest test results on the desk. "You know your brother's condition has been worsening over the past few months." Elena nodded silently. "We've managed to stabilize him tonight, but this is no longer something medication alone can control." She gripped the edge of the chair. "What does that mean?" "It means he needs the next stage of treatment immediately." He slid a document toward her. "The estimated cost is one hundred and eighty thousand dollars." Her ears rang. "...What?" "I'm afraid there's no other option." She stared blankly at the number printed on the paper. One hundred and eighty thousand dollars. It may as well have been one hundred and eighty million. "I..." Her voice cracked. "I don't have that kind of money." The doctor sighed. "I understand." "If I sell everything I own..." She laughed bitterly through her tears. "...I still couldn't pay this." He remained silent for a moment before speaking carefully. "The hospital has agreed to continue treatment temporarily." Hope flickered inside her. "But only for forty-eight hours." The hope disappeared just as quickly. "If payment isn't received within that time..." He lowered his eyes. "...we'll have no choice but to suspend the treatment plan." Suspend. Such a gentle word. Yet Elena knew exactly what it meant. Without treatment... Noah would die. When she was finally allowed to see him, he was asleep. Machines surrounded the hospital bed. A steady beep from the heart machines filled the quiet room. His face looked pale against the white pillow. He seemed so much younger. Not nineteen. Not the boy who always pretended to be strong for her. Just... A child. Elena gently brushed a strand of hair away from his forehead. "I'm sorry." Her voice was barely audible. "I promised I'd protect you." A tear slipped onto the hospital blanket. "I don't know how anymore." She squeezed his hand gently before forcing herself to leave. If she stayed any longer... She would fall apart. Outside... It had started raining heavily. Elena didn't bother opening her umbrella. She simply walked. Cars splashed water onto the sidewalks. People hurried past without noticing the young woman wandering through the storm. She couldn't feel the rain anymore. All she could hear was the doctor's voice. Forty-eight hours. Forty-eight hours to find nearly two hundred thousand dollars. It was impossible. Absolutely impossible. By the time she reached home, dawn was beginning to break. The apartment felt emptier than ever. She didn't turn on the lights. She walked slowly into the dining area and sank into one of the old wooden chairs. The silence pressed heavily against her chest. But something pulled her toward it. Almost as if invisible hands were guiding her. She stood slowly and carried the box back to the table. The brass latch creaked as she opened it. Inside lay pieces of a life she barely remembered. Old family photographs. A faded scarf that still carried the faintest trace of her mother's perfume. Birthday cards written in elegant handwriting. A tiny silver music box. Elena smiled sadly as tears rolled down her cheeks. "I miss you..." She carefully lifted each item. Until... Her fingers brushed against something hidden beneath the bottom of the box. She frowned. "I don't remember this..." Carefully lifting the wooden panel... She froze. There was a cream-colored envelope tucked underneath. Unlike everything else inside the box... It had never been opened. Across the front, written in her mother's unmistakable handwriting, were just five words. “FOR ELENA. ONLY IF NECESSARY.” Elena's heart started racing. With trembling fingers... She slowly broke the envelope seal. Elena's trembling fingers carefully broke the seal of the envelope. The paper was old, with crumpled edges, and it still carried a light lavender scent that reminded Elena of her mother. Tears filled her eyes. It had been six years since she had heard her mother's voice. Six whole years since she had seen her handwriting. Nothing. A third time. Still nothing. By the fifth attempt, an automated message finally answered. "The number you are trying to reach is unavailable." She lowered her head. "No..." She whispered. "This can't be another dead end." For a long moment she simply stared at the number. Her mother had protected this envelope for years. Surely it couldn't end here. Thinking quickly, Elena opened her browser and typed the name into the search bar. Margaret Ashford. Dozens of results popped up immediately. Her eyes widened. Margaret Ashford wasn't an ordinary woman. She was one of New York's most respected philanthropists, known for hosting lavish charity events attended by the city's wealthiest families. Elena scrolled through article after article. Then one headline caught her attention. ASHFORD FOUNDATION TO HOST ANNUAL CHARITY GALA THIS SATURDAY. Elena blinked. Again. And again. She had no designer dress. No connections. No invitation. Nothing. But then Noah's pale face flashed through her mind. The oxygen mask. The machines. The doctor's voice. Forty-eight hours. Finally, she settled on the simplest one. A modest navy-blue dress her mother had bought years ago for a church fundraiser. It wasn't fashionable, neither was it expensive. But it was clean and all she had. She carefully tied her hair into a low bun before wearing a pair of worn silver heels. Looking into the mirror, she barely recognized herself. "You can do this." She whispered. "For Noah." She folded her mother's letter and put it back into the envelope. And stepped out.
