The operating room slowly emptied. The rhythmic beeping of the heart monitor filled the silence. Dr. Isabella Hart stood motionless beside the hospital bed, unable to take her eyes off the man lying before her. Luca. Five years. Five years of unanswered questions. Five years of convincing herself that she had finally moved on. Yet one look at him had shattered every wall she had built around her heart. A nurse approached quietly. "Doctor... should I transfer him to the ICU?" Isabella blinked. "Yes." The nurse hesitated. "There is one problem." "What?" "The men outside won't allow anyone to move him without their permission." Almost on cue, the door opened. A tall man in a black tailored suit stepped inside. He looked to be in his early thirties, with neatly combed dark hair and an unreadable expression. Every movement was calm. Controlled. Dangerous. He stopped a few feet from Isabella. "You are Dr. Isabella Hart?" She nodded cautiously. "I am." He gave a respectful bow. "Thank you for saving our boss." Boss? Isabella frowned. "I only did my job." "You saved far more than one life tonight." His voice remained calm. "If Mr. Moretti had died... a war would have begun before sunrise." A chill crept down her spine. War? Who exactly had Luca become? Before she could ask another question, another man rushed into the room. "Mr. Romano!" "The council is waiting." Romano nodded. "They can wait." "No, sir. The other families have already arrived." He looked back at Isabella. "Our boss requested something before he lost consciousness." Her heartbeat quickened. "What did he say?" Romano's expression softened for the first time. "He said..." "'Don't let Bella leave.'" The room fell silent. Isabella looked away. "He must have been delirious." "No." Romano shook his head. "He said your name very clearly." She swallowed hard. "I haven't seen him in five years." "We know." Her eyes snapped toward him. "You... know?" "We know everything." The words sent a wave of unease through her. Everything? Romano reached into his jacket and placed a small velvet box on the bedside table. "What is this?" "A token of gratitude." "I don't accept gifts from patients." "It's not a gift." "It's a debt." She pushed the box back toward him. "I don't want it." His lips curved into the faintest smile. "Our boss predicted you would say that." Without another word, Romano turned and walked away. The moment he left, Isabella opened the box despite herself. Inside rested a simple silver necklace. Her breath caught. It wasn't new. It was old. Very old. She remembered buying it from a tiny street market when she and Luca barely had enough money to eat. She had given it to him as a promise. *"No matter where life takes us... you'll always have a piece of me."* She thought he had lost it years ago. Instead... He had kept it. A sudden voice broke the silence. "Doctor." She turned. Luca's eyes were open. Weak. Tired. But unmistakably focused on her. Neither of them spoke. Years of pain, unanswered questions, and unspoken love hung between them. Finally, he whispered in a hoarse voice... "I kept my promise." Isabella's eyes filled with tears. "You promised you'd come back." "I tried." "No." Her voice broke. "You disappeared." He closed his eyes. "If I had stayed..." "...you would have died." Before she could ask what he meant, alarms suddenly blared throughout the hospital. A security guard burst into the room. "Everyone down!" "Gunmen are inside the hospital!" Luca struggled to sit upright despite his fresh wounds. His eyes instantly became cold. Deadly. He looked at Romano and gave only two words. "Protect her." Outside... The first gunshot echoed through the hallway. **End of Chapter 2
