Check for the most severe injury and preserve lives. Secure safety and check vital signs. Maybe the kid experience trauma, seeing her parents in the horrible state, it was heartbreaking.
Sunod nilang inasikaso ang mag asawa, the girl is lifeless, she checked the vitals, oxygen and injuries in different parts of her body. Tinawag niya ang dalawang kasamahan.
"Over here! We need to remove her and put her on the stretcher so we can assist further with her health status.
She put oxygen on the female victim. I am losing her; vital signs are weak and she is barely breathing.
I inserted intravenous fluids (dextrose) immediately. I put the vital signs monitor on to check for pulse, breathing, blood pressure, and temperature.
I took the flashlight to check if the eye was constricted. I need to make sure that there is no brain damage or anything. Using light, you can determine if the brain is okay or not. After my assessment, the patient's eyes responded to light. But her vital signs are dropping. Maya-maya nag flat line na ang patient.
"f**k!" Mura nito. "Mike, I need you here. Do the CPR, and I will prepare the defibrillator initial shock of 100 joules and 0.5 mg epinephrine." Malakas kong sigaw! Ayokong mag expired ang patient ko
“Dhee vitals, no changes.” It was Mike reminding me about the patient.
“Open, and let’s start. I will give her another shock. Please put it on the 100 joules and epinephrine shot.
“Mike, concentrate. We had this case before; your eyes are on me. Mike clear out, are we all clear?” I shouted so they could hear me and release my tension.
She put the defibrillator on the right upper part of the sternum and left side lower part of the heart. Pag lapat noon ay tumaas ang katawan ng patient but no changes on the monitor.
“Increase it to 250 joules, 45 amps.” Agad sumunod si Mike.
“All clear."!” Paglapat ng defibrillator sa patient I looks at the monitor, nagkaroon ng changes in vital signs monitor pero mababa pa rin. Pababa ng pababa ang vital signs nito.
“Give me the flashlight, Mike,” Sinuri ko agad ang mata ng patient kong is still not responding to light. Oh God, this is bad.
“Mike make it 360 joules. How many minutes intervals for epinephrine shots?” I asked Mike.
“Michael, how many minutes?” I reminded him again. He was out of focus.
“Five minutes,” Mike responded, his voice trembling. Then I hold his hand and look him in the eye.
“Calm down, Michael, focus, give another shot; we can do this.” Tumango lang ito.
“Dhee given now.” It was Mike in a confident voice. I smiled at him. He wiped the sweat from his forehead. When I heard his go signal, sabay kaming tumingin sa monitor.
“All clear...!Clear!" We spoke in unison.
“Sir, how are you feeling right now? Sir, I need you to fill out the details for your wife, yours, and your daughter.” He is still in shock.
"Thank you, Ma'am, for saving my wife. We owe you her second life.” Maluha-luhang sabi nito sa akin. I was flattered that I could help and save people's lives.
"Sir it’s my job and it’s an honor."
"Come baby, baby mommy is okay now, don't cry, okay?"
“Dhee we are full, pati ang operating rooms natin puno rin. We can't accommodate them. Ilipat mo sila sa Buenavista Medical Hospital nasa two blocks from here.”
"Excuse me, sir, who is in charge here? I have a critical patient; I would like to give my endorsement and health status update."
Doctor Buenavista!" I have a patient here sigaw nito sa doctor."
Damn, what happened to me? Bulyaw niya sa sarili.
"Patient Nicole Fernandez, Married, female, 32 years old, car accident with broken upper torso, broken lower ribs, broken bone in left femur. Possible internal bleeding.
“No head injury, vital signs flat rate for 60 seconds. Defibrillator shock is given from 200 joules for the first shock, 250 joules for the second shock, and 360 joules for the third shock. Epinephrine given in the first initial shot was 0.5 mg, and another 0.5 mg was given after 5-minute intervals. She needs an MRI for a series of tests. Sunod-sunod na paglalahad ko sa Doktor. Here's her chart. All written there based on my assessment and first aid given.”
“If you have any questions, I'll be here." “Patient needs MRI. Prepare the OR? Let me know the health update of our new patient.
Habang binabasa ni Chlyde, patient number two, her husband. No severe injuries, just bruises and scratches. But he has a wound due to a sharp object on his shoulder.
Takang tumingin si Chlyde, “why the passenger was severely injured?" May pagtatakang tanong sa akin. Nagsalubong pa ang kilay niya.
"To answer your question, Doctor Buenavista, the airbag didn't deploy after the car was crushed. As per the two policemen, a light appeared on the dashboard, which means the airbag had been deactivated. You can ask the police about this. That's the only information I had a while ago when I asked the two police officers who arrived first on the scene before we came."
My job comes first.
“Also Doctor Buenavista I need your child's healthcare personnel to assess trauma on the child.”
Karga-karga ito ng ama. Nakatulog na sa sobrang pagod, gutom at kakaiyak
“Nurse please assist these two patients and call childcare services to come here ASAP. Agad naman tiningnan ko ang bata siguro ito ay nasa 4- 5 years old na ito. Parang kinurot ang puso ko…
