Elias first notices her because she is talking about him.

Pain. It came without warning. One moment, Elias Ashford was reviewing a contract, his pen gliding across crisp white paper as the late afternoon sun poured through the glass walls of his office. The next, a sharp pain tore through the center of his chest. His hand froze. The pen slipped from his fingers, rolling across the mahogany desk before falling to the floor with a soft clatter. He inhaled sharply. The air never reached his lungs. His chest tightened, the pressure growing heavier with each passing second, as though an invisible hand had wrapped around his heart and refused to let go. He loosened his tie. It didn't help. Another breath. Nothing. His vision blurred around the edges. "Elias?" The voice came through the intercom. His assistant. "The board meeting starts in fifteen minutes." He opened his mouth to answer. Only silence came out. Cold sweat trickled down his temple. His heartbeat became strange—fast one moment, painfully slow the next. Every beat echoed in his ears like a drum. He stood too quickly. The room lurched. The skyline beyond the floor-to-ceiling windows dissolved into streaks of light. His phone vibrated across the desk. His father's name flashed across the screen. He reached for it. His fingers missed. The phone crashed onto the floor. His knees buckled. Not now. Please... Not now. The office door swung open. "Mr. Ashford?" His assistant gasped. "Elias!" He wanted to reassure him. Tell him he was fine. Instead, the world tilted. The ceiling disappeared. Darkness swallowed him whole. He stopped breathing for a second. Nothing. Beep. Beep. Beep. The sound pulled him back. Elias frowned. His body felt heavy. His chest ached. He tried to open his eyes, but the light was too bright. He closed them again. A few seconds passed. He tried once more. White ceiling. A faint smell of antiseptic. A monitor beside him. An IV in his hand. Hospital. His breathing quickened. A nurse noticed. "Mr. Ashford?" He turned his head. "Can you hear me?" His throat was painfully dry. "Water." The nurse reached for a cup. "Slowly." He took a small sip. His eyes moved around the room. "What happened?" "You collapsed in your office." He remembered the pain. His hand instinctively moved toward his chest. The nurse gently stopped him. "Don't move too much yet." Before he could ask anything else, the door opened. Someone entered. A woman. White coat. Pale blue scrubs. A surgical mask covered most of her face. She walked toward his bed while looking through the file in her hands. "Mr. Ashford?" Elias looked up. And froze. He didn't know why. He couldn't explain it. There was something about her. Something familiar. Her eyes. The way she held herself. The slight crease between her eyebrows as she read. He stared at her. She looked up. Their eyes met. For a moment, the room seemed to become strangely quiet. "Can you hear me?" He nodded. "Good." She stepped closer. "I'm Dr. Reyes. I'll be checking on you." Her voice. There was something about her voice too. He couldn't place it. She checked the monitor, then looked back at him. "How are you feeling?" "Fine." She gave him a look. "You're in a hospital." He said nothing. "You're allowed to say you're not fine." Something about the way she said it almost made him smile. Almost. She continued checking his vitals. "Do you remember what happened?" "Chest pain." "Before you lost consciousness?" He nodded. "Anything else?" "No." She made a note. "We're still running tests, so I don't want you worrying about the cause yet." "Is it serious?" Her eyes met his again. "We're taking it seriously." Not the answer he wanted. But before he could ask another question, someone called her from outside. She turned toward the door. "I'll be back." She walked away. Elias watched her. Something inside him felt unsettled. He didn't know her. At least, he shouldn't. But he knew those eyes. He knew that voice. He knew the way she tilted her head when she was thinking. He just couldn't understand how. The door began to close. "Doctor." She stopped. She looked back. "Yes?" Elias hesitated. For some reason, he wanted to ask her name. Not her professional name. Her real name. The name that belonged to the person behind the mask. "What is your first name?" She seemed slightly surprised by the question. Then her eyes softened. "Isabella." The name struck him harder than the pain had. His breath caught. She didn't notice. "Isabella Reyes." She gave him a small nod. "Try to rest, Mr. Ashford." Then she left. The door closed. Elias stared at it. Isabella. He whispered the name once in his mind. Then again. His eyes slowly closed. He couldn't explain why seeing her had affected him. He couldn't explain why her name felt like something he'd been carrying for a very long time. And he couldn't explain why, after she had walked away, one thought refused to leave his mind. She doesn't remember me. But how did he know that? He opened his eyes again. The room was empty. Only the monitor answered him. Beep. Beep. Beep. Elias stared at the ceiling. For the first time since waking up, he was no longer thinking about his heart. He was thinking about her. And he had no idea why.
