Chapter 10 Bobbi was already seated at the table when Paula walked into the risk manager’s office for the RCA meeting on Thursday morning. Katharine Jenkins welcomed her, and the others at the table nodded. Dave Kellen, the night shift charge nurse, was there already, and so was Melanie, another night nurse. Within a few minutes, Allison and Karen showed up, along with the respiratory therapist who had been on duty during the incident.
