Chapter 1: The Bloody Sacrifice
"Hold his head straight so he can watch every single drop of his precious blood wash down the drain," Vance Vane sneered, his polished leather boot crushing Noah Parker’s left cheek into the filth of the alleyway.
Rain poured relentlessly from the black skies of Oakhaven-on-Aethel, mixing with the hot crimson pooling beneath Noah’s crushed face. The metallic stench of fresh blood filled his nostrils, thick and suffocating.
Every breath felt like dragging broken glass through his lungs. His vision blurred, flickering between the blinding neon lights of Apex Central Hospital above and the abyssal darkness of the storm drain inches from his eyes.
"You really thought you were a saint, didn't you, Doctor Parker?" Vance chuckled coldly, crouching down beside him. "You actually believed your precious oath meant something in our city."
Noah tried to speak, but only a wet, gurgling cough escaped his throat. The cold rain splashed against his split lip, burning the open flesh.
"Look at yourself now," Vance continued, tapping the flat side of a rusted scalpel against Noah’s trembling jaw. "A chief trauma surgeon lying in garbage like the dog you are."
Behind Vance, two massive enforcers from the Vane Clan held Noah’s arms pinned against the wet concrete. Their heavy iron pipes lay nearby, slick with grease and Noah's blood.
The pain in Noah’s legs was an agonizing, roaring furnace. They had shattered both his knees with iron pipes moments ago, the sickening crack of bone still echoing in his ears.
"Why?" Noah choked out, his voice reduced to a raspy, blood-soaked whisper. "It was... a four-year-old child... She had a full life ahead of her..."
Vance’s eyes narrowed into slits of pure, unadulterated venom. He grabbed Noah by the hair and slammed his forehead against the pavement.
"That child’s heart belonged to the Patriarch of the Vane Clan the moment we selected her!" Vance roared into his ear. "Who gave you the right to refuse the harvest?"
"I am a doctor... not a butcher for your dark market," Noah spat, teeth stained red as he forced the words past his tongue.
"You were a doctor," Vance corrected with a sickening smile. "Tonight, you are just an inconvenient piece of meat that needs to be disposed of."
Vance signaled to the enforcer holding Noah’s right hand. The large man dragged Noah’s hand forward, spreading his slender, precise surgical fingers across the hard concrete curb.
Noah realized what was coming and fought with every remaining thread of strength. He thrashed his torso, but his shattered legs were useless weights dragging him down.
"No... please," Noah gasped, terror finally piercing through his physical shock. "Take my life if you must... but not my hands..."
These hands had performed hundreds of delicate procedures, saved countless lives, and held his daughter when she cried. They were his identity, his pride, his entire existence.
"Your hands belong to the underworld now, Doctor," Vance whispered viciously. "Bring the hammer."
The second enforcer stepped forward, hoisting a heavy iron sledgehammer above his shoulder. The dull metal gleamed under the flickering streetlight.
"Stop!" Noah screamed, a raw, ragged sound that tore his throat open. "Vance, I beg you!"
The hammer descended with violent, unforgiving force.
A sickening crunch of shattering phalanges and metacarpals echoed down the narrow alleyway. The agonizing shockwave traveled straight up Noah’s arm, blinding his vision with pure white light.
Noah let out an inhuman shriek of agony, his body convulsing against the pavement as his pointer and middle fingers were flattened instantly.
"That is for disobeying the clan," Vance said calmly, watching the blood spurt from the shattered tissue. "Again."
The hammer swung down a second time. Another horrifying crunch pulverized his ring finger and pinky, splintering the delicate bones into unrecognizable shards.
Noah’s cries dissolved into breathless, high-pitched wheezes. His mind spun on the edge of total unconsciousness, but the brutal pain kept dragging him back to reality.
"Now for the thumb," Vance instructed, his tone as casual as a man ordering dinner. "Make sure he never holds a scalpel again."
The heavy iron head struck a third time, crushing the base of Noah's thumb against the stone curb. Tears of absolute helplessness mixed with the rain cascading down Noah's face.
His brilliant, legendary surgical hands were gone, replaced by mangled, bleeding masses of broken flesh and crushed bone.
"Pathetic," Vance sneered, pressing the point of his rusted scalpel directly into Noah’s right shoulder socket. "Let's make sure the nerves are severed too, just in case a miracle happens."
Vance leaned his full weight onto the blade, slicing deep into the skin and twisting the rusted metal through muscle and tendon.
Noah’s back arched violently, a choked scream dying in his throat as the sharp metal dragged down toward his chest. Fresh, hot blood gushed from the jagged wound, washing over Vance’s leather gloves.
"That should ensure permanent paralysis," Vance said, pulling the bloody blade out and wiping it on Noah’s torn white coat. "You are completely useless now."
The rain continued to fall in heavy sheets, washing the dark blood off the pavement and directing it toward the open storm drain.
"What about the malpractice story?" one of the enforcers asked, leaning on his blood-stained iron pipe.
"Already processed," Vance replied, adjusting his coat collar against the cold wind. "By tomorrow morning, the news will report that Doctor Noah Parker murdered a patient on the operating table while under the influence of illegal narcotics."
"And his daughter?" the enforcer inquired further.
Vance smiled wickedly, looking down at Noah’s half-conscious face. "Little Lily is already in our custody. She will make a fine asset for our underground operations once she grows up."
A surge of wild, hysterical adrenaline fought through the haze of Noah’s pain. "Lily... touch her... and I'll kill you..."
"You can't even stand, dead man," Vance laughed, kicking Noah hard in his fractured ribs. "Throw him into the drain."
The two enforcers grabbed Noah by his ruined coat and dragged his limp, shattered body toward the gaping mouth of the storm drain. His broken legs scraped uselessly across the rough concrete.
They hoisted him over the iron ledge without a second thought.
Noah plummeted into the freezing, filthy darkness below, hitting the turbulent underground currents with a heavy splash. The foul water swallowed him instantly, pulling his heavy, broken body down into the deep subterranean sewers.
Submerged in the freezing sludge, the extreme cold hit his open wounds like thousands of needles. His lungs burned for air as the current dragged him through the dark tunnels.
He forced his head above the water for a brief second, gasping a lungful of foul air before the rushing water pulled him under again.
I can't die here... Noah's mind screamed through the agonizing haze. Lily needs me... I have to live...
Using his elbows and remaining upper arm strength, he desperately clawed at the slippery brick walls of the sewer pipe, trying to find a hold. His shattered fingers dragged uselessly against the slime, sending sharp spikes of agony through his central nervous system.
He managed to hook his left elbow over a rusted metal pipe jutting out from the side of the sewer wall. He pulled himself upward inch by painful inch, coughing out mouthfuls of sewage and blood.
His body hung suspended over the roaring black water, trembling uncontrollably from hypothermia, shock, and blood loss.
Every nerve ending in his hands burned as if dipped in acid. His ruined shoulder throbbed with every heartbeat, spilling more life out into the dark waters below.
He had lost his medical license, his reputation, his pristine hands, and his innocent daughter in a single night. The Vane Clan had taken everything and left him to rot in the city's stomach.
Noah squeezed his eyes shut, pressing his forehead against the icy sewer wall as tears of pure, unadulterated hatred cut through the grime on his face.
He wouldn't die in this filth. He would survive, no matter how much bone he had to crawl over, or how much blood he had to spill.
Suddenly, the heavy iron grate directly above his head rattled in the dark street above.
Heavy, methodical footsteps echoed through the storm drain, stopping directly over the position where Noah hung suspended in the dark.
A bright beam of a tactical flashlight cut through the iron grate, blinding Noah’s soaked, bloodshot eyes.
"Is he down there?" a harsh, unfamiliar voice barked from above the grate.
"Find him and make sure his heart has stopped beating," a second voice cold as ice commanded. "Vance wants a picture of his corpse before sunrise!"
