The smell of antiseptic and cheap coffee was supposed to be my safe haven. As a senior trauma nurse, I didn't have time for fear. I didn't have time for anything except the rhythmic beep-beep-beep of cardiac monitors and the sterile rush of saline. I was finishing up my third back-to-back twelve-hour shift, re-wrapping a teenager’s sprained wrist, when the automatic double doors of the ER bay didn't just open—they slammed inward with a violence that rattled the safety glass. "Clear the room! Now!" a voice boomed across the lobby. Three men in tailored black suits stormed past the security desk. Security didn't stop them. One look at the heavy silver pistols concealed under their lapels kept the guards frozen, hands hovering nervously near their holsters. In the center of the trio was a towering man, his arm slung heavily over the shoulders of a scarred subordinate. Even half-conscious and dripping blood onto the linoleum, the man in the middle radiated a terrifying, suffocating authority. Broad shoulders stretched the ripped remnants of a custom charcoal suit. Dark, razor-sharp jawline stubbled with night-shadow. And when he lifted his head, his eyes caught the fluorescent lights—piercing, ice-cold gray, filled with a lethal, unyielding calm despite the bullet hole in his side. "Doctor!" the lead bodyguard barked, slinging the heavy man onto Trauma Bay 3’s steel table. "Where is the head surgeon?" "He's in emergency surgery," I said, stepping forward automatically, my hands snapping into latex gloves before my brain could process the danger. "I’m the charge nurse on duty tonight. What happened?" "He doesn't need a nurse," the bodyguard growled, stepping into my personal space. Up close, I could see the distinct silver dragon tattoo crawling up his neck. "Get us a surgeon, or I’ll shut this entire unit down." "He doesn't have time for a surgeon," I snapped, brushing past him with practiced authority. "He’s in hemorrhagic shock. His blood pressure is dropping, and if I don't stop the arterial bleed right now, he'll be dead before anyone gets down from the fifth floor." The giant on the table let out a low, raspy chuckle. It sounded like grinding stones. "Listen to the nurse, Marco," the stranger rasped. His voice was dark velvet wrapped in grit. "She’s the only one in this room with any sense." I didn't waste another second. I cut through his blood-soaked silk shirt with trauma shears, revealing a wall of lean, corded muscle covered in jagged white scars—and a fresh, pulsing gunshot wound beneath his left rib cage. The bullet had missed his heart by inches, but it had nicked an artery. Blood was pooling fast. "I need two units of O-negative, a suction catheter, and a vascular clamp," I called out to the empty hallway, but the nurses outside had been pushed back by two more armed guards stationed at the doors. I was on my own. "Hold him down," I ordered Marco. "Nobody holds down Matteo Moretti," the bodyguard warned. Matteo Moretti. The name hit my ears like a bucket of ice water. The Moretti Syndicate. The undisputed rulers of Chicago’s underworld. Every citizen in the city knew the name, but few saw his face and lived to talk about it. I looked down at the man lying under my hands. The cold Don of Chicago was staring up at me, his intense gray eyes tracking every micro-expression on my face. His hand shot up, wrapping around my wrist with the grip of a steel vice. The heat of his palm burned through my latex glove. His knuckles were bruised, his grip tight enough to stop my own circulation, but I refused to break eye contact. "You're hurt, Mr. Moretti," I said, my voice steady despite the adrenaline coursing through my veins. "If you snap my wrist, I can't save your life." For three agonizing seconds, we held each other’s gaze. I saw something shift in his storm-cloud eyes—a flicker of dark intrigue behind the pain. Slowly, his fingers uncoiled from my arm, leaving red indentations behind. "Bold little bird," he whispered, a faint, dangerous smirk touching the corner of his lips before his eyes rolled back, and his body went limp. Forty minutes later, the bleed was stabilized, the bullet removed, and his vitals were holding steady. I wiped my brow with the back of my arm, my green scrubs entirely soaked in Matteo Moretti’s blood. I stepped away from the table to write down his vitals on the chart, taking my first real breath in an hour. "He's stable," I told Marco, who stood near the doorway like a stone monolith. "The trauma team can take him to ICU now. You can take your men and go." Marco looked at his watch, then down at his encrypted phone. "No ICU." "Excuse me?" I turned around, incredulous. "He just survived major vascular surgery in an ER bay. He needs intensive monitoring—" "The Don doesn't stay in public hospitals," Marco interrupted coldly. "It’s not safe. The men who shot him know he came here." Before I could object, the double doors swung open again. Four more suited men rolled a private transport gurney into the room. Within seconds, they transferred Matteo’s unconscious, strapped-down body off the hospital bed. "Wait!" I yelled, reaching for the gurney. "You can't move him! His stitches will tear, he’ll bleed out internally—" Marco stepped between me and the gurney. He didn't draw a gun; he didn't need to. He simply placed a heavy hand on my shoulder and shoved me gently backward into the transport group. "You're right, Nurse Vance," Marco said, reading my ID badge for the first time. "He does need intensive monitoring." "Which is why you need to let the staff—" "Which is why," Marco cut in, "you’re coming with us." My heart slammed against my ribs. "What? No! I’m on duty! I have patients—" "Not anymore." Two guards grabbed my arms, hauling me toward the back exit where a fleet of blacked-out SUVs sat idling in the rain. I kicked and thrusted against their iron grip, but my 5-foot-6 frame was no match for 200-pound enforcers. They threw me into the leather back seat of the lead SUV. A second later, Matteo was loaded into the rear, and the doors slammed shut, locking with a heavy electric click. The vehicle accelerated violently into the stormy night. In the dim interior light of the back seat, I sat trembling, my heart hammering against my chest like a trapped bird. Next to me, a low groan echoed from the medical gurney. Matteo Moretti was stirring. His gray eyes fluttered open, heavy with residual anesthesia, but instantly focusing on me. He looked at my blood-spattered scrubs, then up at my terrified face. "Where... are we?" I choked out, pressing my back against the locked passenger door. "Tell them to let me go! I saved your life!" Matteo stared at me for a long, silent moment. Slowly, painfully, he reached out his uninjured hand, his long, scarred fingers cupping the back of my neck and pulling me inches from his face. His hot breath brushed my cheek, smelling of iron and mint. "You saved my life, little nurse," Matteo rasped, his voice smooth, dark, and utterly merciless. "And now you know what my blood looks like. You know what my face looks like." "I won't say anything to anyone!" I pleaded, tears threatening to spill. "I promise!" Matteo’s thumb stroked the frantic pulse point at my jawline, a chilling smile touching his lips. "I know you won't," he whispered softly. "Because you're never leaving my side again."
