Options Ren The woman was taken out of the bus by the paramedics. Almost every injured was out of it but the two man. Their condition was critical and was worsening with every second passing. The temperature was hiking as the noon was approaching. The sun was rising high to the horizon and the bus was getting sweaty. I went to the middle-aged man, who was lying unconscious. “did the men arrived?” doctor Ezekiel asked. “This man can't take any more pressure on his chest.” He added. “we can't life the bus, he will die.” I said panicked. My panicked voice awoke him up. “please save me…. please save me.” he uttered weakly. The man has his legs under the other side of the metal plate. If it was to be removed or lifted, he won’t be able to survive. “my leg….” His weak voice came filled excruciating pain. “don’t move please. Don’t use your energy.” I warned him holding his thighs. “we have to lift the ceiling, or this patient will pass away. He doesn’t have much time. His pulse rate is dropping. He is cold.” Doctor Ezekiel said again. “no, we can't do that. This patient will die.” I insisted. “his two legs have been trapped for thirty minutes. There is a possibility for Crush Syndrome!! (Major shock or kidney failure after crushing an injury to the skeletal muscle.) if this is removed, he could go into cardiac arrest.” I continued. “we need to removed it slowly while we administer IV fluids.” “it will be more dangerous for him; he is getting cold.” Ezekiel said. “if we lift it now, or this patient will have a cardiac arrest.” I declared. “if we don’t lift it now, this patient will expire!!” Ezekiel persisted. “please save me, my son… my son…” the man again muttered. - - - - - - - - - - “Mrs. Ann, is there still no news from the scene?” doctor Norah asked to the head nurse in the trauma centre. “no, the rescue operation must be going slowly.” She replied. “there are three doctors at the scene, why are you worrying about it?” anesthetic doctor said standing near them. “are you not preparing for the operation?” nurse asked. “I'll take care of my business on my own. Don’t worry about that.” She said coldly looking in her eye. - - - - - - - - - - The last one was finally taken out of the bus. “doctor I can't feel anything in my legs.” The patient said groaning. “is there sodium bicarbonate in there?” I asked the paramedic. “I don’t see it.” “let’s put a line in for now and start the fluids.” I suggested. “are you trying to give up on this patient?” doctor Ezekiel said. He was angry and his voice was loud, almost yelling. “it’s impossible to save both patients, we have to choose. And this one is still conscious.” I spoke. “how long it is going to lift this?” Ezekiel asked the medic. “how long will it take?” he asked another. “the preparation is almost done.” the ceiling was to be pulled by the crane so the metal will be lifted with it. “intern go there. Wrap his legs with elastic bandages first.” Doctor said. Kohen came to me with bandaged in his hands. “then we will lift the bus.” He said. “doctor what are you trying to do? he will die, it’s obvious that the crush syndrome will be brought on.” I insisted. “It could come or it might not.” “are you willing to undertake this risk with an uncertain possibility of success.” I asked. I was disappointed. I have been trying to be practical, we cannot always save everyone. It will only risk the life of this man. “because of the probability that isn’t even certain, I can't give up someone who is still living.” He stated. “then both could die.” I stopped the intern. “they are both alive and I will save both of them.” He pushed me away, making his voice pierce through my head. I thought he was crazy. Either he is stupid or just a liar. But this person…. “get it ready. I will give you the signal. Raise it by thirty centimetres.” He ordered. “is the crane ready?” the man asked. “yes. Is everyone okay inside?” another asked back. “yes, I will give you the signal, then raise it by thirty centimetres.” “okay, understand.” “intern take care of the chest tube.” He said and ran to the patient I was with. “can you do that?”
